Lost Urgent / Ordinary Passport- Requirements

• 06 Passport size (4x5cm) front pose photograph with light blue background. Photograph must be not old more than 06 months.
• Original CNIC or NICOP with a clear photocopy.
• Dully filled in application form “A” signed and with thumb impression on 03 specified places.
• Copy of lost passport if available.
• Verification form to be sent to issuing authority for verification of lost passport.
• Affidavit with sponsonr’s letter, police report and report of Jawazaat called “Shahada badal Faqit” signed and stamped with photograph of the applicant.
• Announcement of lost passport in any newspaper of Saudi Arabia

Damage / Wet Urgent / Ordinary Passport- Requirements

• Original Passport.
• 05 Passport size (4x5cm) front pose photograph with light blue background. Photograph must be not old more than 06 months.
• Original CNIC or NICOP with a clear photocopy.
• Dully filled in application form “A” signed and with thumb impression on 03 specified places.
• Verification form to be sent to issuing authority for verification of  damage passport.
