Procedure for Addition of Child on Passport
- Original passport of mother with a copy.
- Form “C” to be filled by the mother.
- Two passport size photos of child.
- Copies of parents CNIC/NICOP.
- Original Birth Certificate issued from the concerned Ministry, Saudi Arabia and verified by Ministry of Foreign Affairs, Saudi Arabia.
- If father is foreign national the children cannot be endorsed on mother’s passport.
- If mother is foreign national the children can be endorsed on farther’s passport provided copy of mother’s passport is enclosed.
Procedure for deletion of child from mother/father’s passport for issuance of fresh passport.
- Original passport of mother with a copy.
- Form “C” to be filled by the mother.
- Form “A” for new separate passport for the child with 4 passport size photos.
- Copies of parents CNIC/NICOP.
- Original Birth Certificate issued from Ministry of Health, Saudi Arabia.
- If father is foreign national the children cannot be endorsed on mother’s passport.
Procedure for amendments in existing valid Pakistani passports.
- A-Change in holders name/father’s name, date or place of birth
- Changes in name/father’s name or date/place of birth will be done according to CNIC/NICOP supported by educational documents, Press cutting etc. with an application on Form c.
- For endorsement of husband name in the passport of the lady. Form c with original Nikkahnama and a copy verified from MOFA Pakistan or Saudi Arabia, as the case may be, original CNIC/NICOP, Passports of both the husband and wife with copies are required.
- Please note that no amendment or endorsement is allowed on Machine readable Passports.
